Documents Mandatory to Operate as New Customs House Agent (Declarant)

  1. Request Letter (Signed by the Proprietor/Partner/Director)
  2. Printout obtained after submitting all required information and e-documents to e-registration web portal (https://ereg.customs.gov.lk/registrations/login)
  3. Originals with Photocopies of following Documents
    • Business Registration Certificate (BRC/ Form 2A/ Form 41/ Form 42)
    • TIN
    • Form 1/ Form 48/ Form 40 (Obtained from Company Registar – if applicable)
    • Articles of Association/ Memorandum of Association
    • Shipping Agents License issued by Director Merchant Shipping (Only for Shipping Agents)
    • Deed or Lease Agreement
    • Electricity Bill (CEB or LECO – within two months)
    • Google map location of the company
    • Google Street view image
    • NIC/ Passport of Directors/ Partners/ Proprietors/ Wharf Clerks
    • Certificate of Competence
    • GS Form – 01 GS_Form_01.pdf
    • GS Form – 02 GS_Form_02.pdf
  4. Inventory list with all the serial of electronic equipments and office setup Plan
  5. Cash Guarantee for Rs. 50,000/= deposited in Sri Lanka Customs
  6. Duly filled CHA Application form along with Originals & photocopies of following documents
  7. Cancellation report of previous CHA pass
  8. CIU Reports

Documents Mandatory to Renew Customs House Agent

    1. Request Letter (Signed by the Proprietor/ Partner/ Director)
    2. Duly filled CHA Application Forms
      • CUS/ CHA (01) – One Form
      • CUS/ CHA (02) – Two Forms
    3. Originals with Photocopies of following Documents
      • Business Registration Certificate
      • TIN
      • VAT
      • Cash Deposit receipt of Rs. 50,000/= issued by Sri Lanka Customs
    4. Letter of Certificate issued by the Bank (Bank should issue a letter certifying current account is in operation in the name of Custom House Agent)
    5. Electricity bill ( Within last two months )
    6. Following documents of CHA Applicant’s along with Originals & Photocopies
      • National Identity Card
      • Previous year CHA Pass
      • Certificate of Competence (CHA Certificate issued by Sri Lanka Customs)
      • CIU Report
      • Grama Niladari Certificate – GS Form 01 (within six months)
